Nursing Home Transition & Diversion (NHTD)

Helps Medicaid-eligible seniors and people with physical disabilities live in community settings with comprehensive supports.

Participants are the primary decision-makers and select their services, providers, living setting, and personal outcomes. Services may include service coordination, home and community supports, personal care, environmental modifications, and assistive technology tailored to your goals. CitiHealth NY partners with you and your family to create a person-centered plan that maximizes independence while ensuring health, safety, and community inclusion.

Eligibility (NHTD)

  • Assessed as needing nursing home level of care
  • Medicaid recipient eligible for Community-Based Long-Term Care services
  • At least 18 years of age
  • Safe, adequate community housing
  • Needs can be met within available Waiver and Medicaid resources
  • Require at least one waiver service per month to assure health and welfare

Traumatic Brain Injury (TBI) Waiver

Supports individuals with a TBI—or similar non-degenerative condition—to live in the community with needed services.

Eligibility (TBI)

  • Medicaid recipient; ages 18–64 at application
  • Choose community living vs. nursing facility
  • Primary diagnosis of TBI or similar condition (e.g., stroke, anoxia)
  • Assessed as needing nursing home level of care
  • Approved service plan ensuring safety in the community

General Waiver Eligibility

  • Need nursing home level of care
  • At least 18 years of age
  • Safe, adequate community housing
  • Eligible for Medicaid Community-Based Long-Term Care
  • Needs met within waiver/Medicaid resources
