Helps Medicaid-eligible seniors and people with physical disabilities live in community settings with comprehensive supports.
Participants are the primary decision-makers and select their services, providers, living setting, and personal outcomes. Services may include service coordination, home and community supports, personal care, environmental modifications, and assistive technology tailored to your goals. CitiHealth NY partners with you and your family to create a person-centered plan that maximizes independence while ensuring health, safety, and community inclusion.
Rights to self-direction, responsible risk-taking, and learning—within a framework that protects health and welfare.
Supports individuals with a TBI—or similar non-degenerative condition—to live in the community with needed services.
Supervision and/or assistance with activities of daily living—while fostering independence and participation.
Locate, obtain, and maintain housing that supports safe, independent community living.
Through Home and Community-Based Services (HCBS) Waiver Housing Programs, participants receive help in locating, obtaining, and maintaining housing that supports independent community living.
